Adoption Application

PLEASE READ: You are not buying a dog, you are making a donation. As such, all donations are non-refundable.

This application process is for us to know as much about you as possible to determine whether you and the pet you are applying form are a good fit. The information you provide on our application/contract will help us make that decision. Please answer all questions honestly, as there are no right or wrong answers. If the application is not fully completed it will be discarded without notice. If you have a question or the feel the question does not apply, fill the blank with N/A.

NLDR relies on your donations to care for all the animals in our care. Adoption donations range from $100.00 to $400.00. This donation helps defray the costs of veterinary care given to your dog before adoption. This care includes some or all of the following, but is not limited to, all vaccinations, heartworm testing and prevention medication, flea prevention, fecal testing and worming if the necessary and surgical procedure for spay/neuter. Your adoption donation also helps to feed, house and provide veterinary care for the dogs still waiting to be adopted and as such is NON-REFUNDABLE.

This agreement is made between New Life Dog Rescue Inc. (further known as NLDR) and the adopter. By signing below, you are acknowledging receipt from NLDR, the dog described within the application. Adopting party must be 21 years of age or older and agree to be bound to and comply with the following terms and conditions:

I understand that a home visit by an NLDR representative or fellow rescue group representative is required as a condition of this adoption approval.

I understand that written approval and proof of deposit payment will be required from a landlord as a condition of this adoption.

I will have this dog seen by a licensed veterinarian of my choice, at my expense, within 72 hours from the date of this contract, with consideration and potential waiver of this individual clause of this agreement by an authorized member of NLDR if recent veterinary care has been provided while under the supervision of the foster parent applying to adopt the aforementioned pet or by the adopting party in pre-adoption arrangements. Thereafter, I will have this dog examined at least once a year by a veterinarian and immunized as recommended by that veterinarian, at my expense.

I will seek appropriate veterinary care and/or treatment for this dog if he/she should become sick or injured.

I will provide a loving, safe, happy, clean and gentle home for my new pet at all times.

I will provide proper and sufficient food and water for my pet at all times. I understand that premium pet foods are better for my dog and that certain grocery store brands have been linked to diseases in animals. I will make every effort to provide the best food available at least twice a day.

I understand that I am adopting an indoor dog and will make him/her an indoor member of my family. This animal shall never be maintained outside as a yard dog and shall never be staked out or chained.

I will keep my pet parasite and flea free and will use a veterinarian recommended product such as Advantage, Frontline or Revolution monthly. Generic store brands, such as Hartz, can be harmful or deadly.

I will keep my pet on a monthly heartworm preventative recommended by my veterinarian.

I will not permit this animal to run at large or become a public nuisance and will always keep means of identification on him/her at all times. I will immediately retrieve this animal from any public or private facility when notified. In the event this animal becomes lost I will make every reasonable effort to relocate and claim this animal including notifying NLDR.

I have been advised to clean my pet’s ears and teeth regularly and to trim his/her nails. I will discuss this with my veterinarian if I have any difficulties.

This animal will not be transported in an open pickup truck or car at any time.

No one in my home, including myself, had or now has a pet-related allergy that has not been disclosed to NLDR.

If I can no longer keep or care for my pet at any time during his/her life, I agree to contact NLDR. NLDR will only be able to take the dog back based on foster discretion and availability. If I return this animal to NLDR for any reason I will at no time assert any claim, charge or demand of any kind or nature against NLDR for any charges that may have been incurred by me, including veterinarian fees, in connection with this animal.

I will not use, sell, or donate by the pet for research purposes and understand that ownership of this pet may not be transferred to any other person, firm, corporation or organization for any reason whatsoever.

I understand that an animal can be unpredictable and NLDR cannot anticipate or insure against the unexpected conduct of an animal adopted from NLDR. I acknowledge that NLDR has not made through its volunteers, any warranties regarding the future condition, temperament or conduct of this animal. I hereby accept this animal as is, assume all risks and responsibilities associated with ownership of this animal, including bites and I hereby fully and completely release, indemnify, and hold harmless, NLDR, its directors, volunteers and agents from any claim, cause of action or liability of any sort or nature, whether known or unknown, directly arising out of or in connection with the adoption, care or ownership, maintenance, retention, temperament, conduct or condition of this animal.

I agree to communicate with NLDR during the new pet adjustment period to ensure that a mutually satisfactory relationship is established in the new home.

I agree to make a donation for each puppy/dog adopted. Donations range from $100 to $400 depending on breed/size and veterinary/medical care received. I understand that a fee of $45.00 and or immediate return of this animal, as deemed by an NLDR Director, will be incurred upon the receipt of an insufficient check. Cash donations are preferred and this contract will serve as a receipt with the donation amount indicated.

I understand that my adoption donation is non-refundable.

This contract may be specifically enforceable by NLDR through judicial proceedings, including the right of NLDR to recover the animal due to any breach of any terms of this contract. I hereby agree that in the event I breach this contract and NLDR files suit to enforce this contract, or to defend any claims under this contract, I will pay any court costs and attorney’s fees incurred by NLDR in connection herewith. Any court proceeding will be conducted in Hillsborough County, Florida.

I understand that this is considered a signed contract.

This application for adoption is null and void as a contract to adopt the aforementioned pet until it is received, reviewed, approved and signature of final acceptance is conducted by an authorized representative of New Life Dog Rescue Inc. A copy of the final signed document will be provided to the adopting party.

No modifications, changes, deletions, additions, alterations or omissions may be made to this binding contract without explicit awareness, consent, approval, and documentation by the adopting representative and member of New Life Dog Rescue Inc. regardless as to who initiates or executes these modifications. Only an acting member of New Life Dog Rescue Inc or a designated legal representative acting on behalf of New Life Dog Rescue Inc. may initiate and execute any such modifications.

If the adopting party is unwilling or unable to complete, initial and sign in the designated areas of this adoption application and agreement, the aforementioned pet will remain the legal and virtual property of New Life Dog Rescue Inc. The adoption donation, if already donated to New Life Dog Rescue Inc. and only that exact amount, may be returned to the adopting party at the discretion of the acting member of New Life Dog Rescue Inc. less any fees that may have been incurred by or assessed to New Life Dog Rescue Inc. if required or initiated to facilitate the return of the aforementioned pet to the care and legal guardianship of New Life Dog Rescue Inc. Such fees may include, yet not be limited to, transportation, boarding or kenneling fees for the aforementioned pet or any fees required for legal counsel and/or court proceedings.

In the event of such disagreement or dispute, this contract will be fully binding and enforceable and all efforts will be made to return the aforementioned pet to New Life Dog Rescue Inc. as expediently as possible and without undue duress or delay.
